Services

Your Health Narrative provides professional documentation support, including organizing medical records, developing medical timelines and narratives, and preparing documentation to support communication with healthcare providers and other organizations.

These services are administrative and informational in nature. I do not provide medical, legal, or insurance advice, diagnose medical conditions, interpret medical records, or guarantee any outcome related to healthcare, disability claims, insurance coverage, or legal matters.

Client Responsibilities

Clients are responsible for providing complete and accurate information to the best of their knowledge.

While I take great care in preparing documentation, clients are responsible for reviewing all materials before they are submitted or shared with any healthcare provider, insurer, employer, attorney, school, or other third party.

Consultations

Complimentary consultations are intended to determine whether my services are a good fit for your needs. A consultation does not establish an ongoing client relationship or obligate either party to move forward.

Pricing & Payment

Project pricing is based on the agreed scope of work. A written quote will be provided before work begins.

Payment terms will be communicated prior to the start of each project. If additional work is requested beyond the original scope, a revised estimate may be provided before that work begins.

Turnaround Times

Estimated timelines are provided in good faith and may vary depending on project complexity, record volume, client responsiveness, and other unforeseen circumstances. If delays arise, I will communicate them as promptly as possible.

Revisions

Reasonable revisions related to the original scope of the project are included unless otherwise agreed. Requests that substantially expand the scope of work may require an additional quote.

Confidentiality

Your Health Narrative your privacy is a priority. Information shared with Your Health Narrative will be used solely for the purpose of completing your project and will be handled with appropriate care.

While reasonable safeguards are used to protect your information, no method of electronic communication or data storage can be guaranteed to be completely secure.

Intellectual Property

Clients retain ownership of their medical records and personal information.

All original documents, templates, formatting, and materials created by Your Health Narrative remain the intellectual property of Your Health Narrative unless otherwise agreed in writing.

No Guarantee of Outcomes

Your Health Narrative helps organize and communicate existing information but cannot guarantee any specific medical, legal, insurance, disability, employment, or educational outcome. Final decisions are made by the appropriate organizations and professionals.

Limitation of Liability

To the fullest extent permitted by law, Your Health Narrative shall not be liable for any indirect, incidental, special, or consequential damages arising from the use of this website or services provided.
